Services Provided by Caregivers in the Home

Our Registered Nurses and Registered Practical Nurses provide a wide range of condition-specific care tailored to the unique needs and abilities of your loved one.

Health Assessments

A registered nurse will identify the unique medical, psychosocial, and functional capabilities of your loved one, create a coordinated care plan and identify any needed in-home caregiver duties to help maximize their overall functional ability, physical health, and mental wellness.

Our comprehensive health assessment includes the following:

Functional abilities

  • Activities of daily living (ADL)
Our in-home nursing caregivers in Toronto, ON, are specially trained to provide as-needed support for activities related to personal care (e.g., eating, dressing, bathing, transferring between bed and chair, using the toilet, and controlling bladder and bowel functions).
  • Instrumental activities of daily living (IADL)
Our in-home caregiving duties also include as-needed support for more complex tasks (e.g., doing housework, preparing meals, taking medications correctly, managing finances, and using the telephone).

Comprehensive health history

  • Current illnesses
  • Past and present medical issues
  • Family and social history
  • Demographic data
  • Symptom review

Physical health observation

  • Screening for disease
  • Nutrition
  • Vision
  • Hearing
  • Urinary continence
  • Balance and fall prevention
  • Osteoporosis
  • Drug interactions

Post-Operative Care

The risk for at-home falls, mismanaged medication, and changing pain management needs can significantly increase following a medical procedure.

When your loved one is discharged from the hospital, our Registered Nurses and Registered Practical Nurses will design a post-operative care plan, often including caregivers and in-home services, tailored to their unique needs and abilities so they can recover safely—and more comfortably—in the comfort of their home.

We provide 24-hour in-home nursing caregivers for the elderly as needed, as well as several in-home senior care services, including:

  • Pain, nausea, and other post-operative symptom management
  • Medication management and administration
  • Surgical site and drainage tube care and management
  • Rate and patency (free of blockage) of IV fluids and IV access maintenance
  • Vital sign monitoring to quickly intercept potential issues
  • Patients’ level of sensation and circulation monitoring to quickly address potential issues
  • Other condition-specific recovery support as needed

Care Training for Family Members

Our Registered Nurses and Registered Practical Nurses have extensive in-home care provider experience caring for aging loved ones and guiding family members on how to safely care for them at home.

We start by redefining your role as a caregiver, helping you understand that you are more than just a son, daughter, niece, or nephew. You are now a caregiver, too. As part of our assessment, you can expect a º£½ÇÉçÇø Nurse or our professional in-home caregivers in Toronto, ON, to guide you in the following:

  • Home safety
Learn fall prevention, fire safety, infection control, medication safety, emergency preparedness skills, and how to provide a safe environment.
  • Body mechanics and positioning
Discover how to prevent injuries and ulcers with proper bed positioning, transferring from bed to chair (or chair to
car), and walking assistance techniques.
  • Personal care tasks
Understand how to provide dignified care for daily tasks such as mouth care, grooming, dressing, bathing, showering, and toileting.
  • Healthy eating
Learn about your loved one’s special dietary needs and how to help them prepare for a meal and eat. Find out how to respond to choking, why fluids are essential, and how best to keep food safe.
  • Alzheimer’s or dementia care
Learn how best to support and comfort your loved one.

Medication Administration

Our Registered Nurses and Registered Practical Nurses work closely with primary care doctors and specialists to ensure your loved one’s medications work optimally and control symptoms as expected. If anything changes, they will contact family caregivers, professional in-home caregivers, and care team providers to quickly reassess your loved one’s medical needs and ensure all caregivers are informed of the new medications and dosage.

We commonly manage the administration of oral and topical medications, eye and eardrops, injections, inhalants, oxygen, IVs, blood draws, and wound care.

Vital Sign Monitoring

Consistent vital sign monitoring helps our skilled Registered Nurses and Registered Practical Nurses assess your loved one’s well-being and detect underlying health conditions early. Vital signs are critical indicators of a person’s health and current medical status.

As part of our daily care tasks, our team monitors your loved one’s body temperature, pulse or heart rate, respiratory rate, blood pressure, and oxygen level/saturation and notifies the care team of any changes.
